Medical Records and Privacy Statement
We keep a detailed record of the healthcare services we provide to you. We do our best to be accurate. You may ask to see a copy of your record. You may also ask to correct that record. Please note that it will show up as an edited copy.
We will NOT disclose your record to others unless you direct us to do so or unless the law authorizes or compels us to do so. Legal authorization includes releasing your medical information to any physician or healthcare practitioner to whom we refer you, and to any payer of your care, including your health insurance or managed care provider, upon their specific request.
All of our employees and providers have agreed to and signed a strict confidentiality policy.
We are happy to provide you with a copy of your medical records from our office. A proper release form must be signed. We need at least 48 hours notice for specific medical records and two weeks notice for your complete chart. A fee may be applied. In some instances we may be able to include your records that have been forwarded to our office from other sources, but the expected standard is limited to those records generated from this office only.
